
If you would like to join our surgery please print off the GMS1 form and the appropriate registration form (adult or child) and complete with black ink, signing the Practice Agreement page. You also need to read, print & sign the Summary Care Record Consent Form. Please note if you are registering a child using the appropriate form we will need to see a copy of their birth certificate, thank you.

You do not need to provide proof of your address, photographic ID or immigration status to register with us.
Please note: If you have provided a UK mobile number, we will text you a welcome message once we have accepted and added you to our system. We aim to get new patients registered within 7 working days after your submission. If you do not receive a text after 7 working days, please contact us to find out if you are registered. If there is a problem with your registration, we will contact you as soon as possible to ensure there are no delays in registering you
Practice registration policy
Leander Family Practice complies with NHS England Patient Registration Standard Operating Principles for Primary Medical Care.
Any individual in England may register with a GP without charge. The length of time or reason for the registration will determine the type of registration at the practice e.g. Permanent, temporary or immediately necessary.
All individuals wishing to register with Leander Family Practice as a permanent patient will be required to complete a New Patient Registration form.
It is practice policy to request identification from all new patients as this will ensure the correct matching of details to the NHS Central Registry. The practice has a list of examples of acceptable identification. Registration documents will be stored securely in the patient’s paper medical record.
The inability to provide identification will not result in the delay or refusal to register at the practice.
Individuals will be given the option of registering as temporary or immediately necessary if they are residing in the practice area for more than 24 hours but less than 3 months. Immediately necessary provision will be for 14 days.
Leander Family Practice has a practice boundary and anyone wishing to register as a patient needs to reside within the boundary or for temporary patients be temporarily resident at an address in the practice area.
Any existing patient who moves to an address outside the practice boundary will be required to register at another practice. Homeless or individuals with no fixed abode will be able to register with Leander Family Practice, arrangements will need to be put in place to be able to contact the patient with results.
Leander Family Practice does not discriminate in the registering of new patients on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition. Refusal to accept an application to register will not be made without reasonable grounds e.g. the patient lives outside the practice area or the practice list is closed to new patients.
People with Insecure Immigration Status Everyone in England, regardless of their immigration status, is entitled to free primary care and to register with a GP. This includes temporary visitors/ tourists, asylum seekers, refugees and other vulnerable migrants. A patient does not need to be ‘ordinarily resident’ in the country to be eligible for NHS primary medical services – this only applies to secondary (hospital) care. In effect, therefore, anybody in England may register and consult with a GP without charge
